In the past 10 years, semen quality of Slovene men from infertile couples has deteriorated. The bias level for patient inclusion decreased: this was a cohort study conducted in one country over the study period. The same methods of semen analysis were used by the same three technicians and our reference centre is part of a centralized health sytem in which infertility management is covered by social insurance. Therefore, it is assumed that only a small number of patients were excluded. In addition to negative environmental effects on semen quality, our study also raised some questions about the role of socioeconomic factors (access to and quality of medical care and information on male infertility). Deterioration of semen quality can also be the result of a higher level of confidence in local hospitals, who tend to refer more difficult male infertility cases to our clinic.
