Immediate breast reconstruction (IBR) rates after mastectomy are increasing. Postmastectomy radiation therapy (PMRT) contouring guidelines for target volumes in the setting of IBR are lacking. Therefore, many patients who have had IBR receive PMRT to target volumes similar to conventional simulator-based whole breast irradiation. The aim of this paper is to describe delineation guidelines for PMRT after implant-based IBR based on a thorough understanding of the surgical procedures, disease stage, patterns of recurrence and radiation techniques. They are based on a consensus endorsed by a global multidisciplinary group of breast cancer experts.

COBISS.SI-ID: 3245179Surgery, irradiation, hormones, cytostatics and / or biological drugs often cause the patient to experience co-occurrences and late sequelae. Surgical treatment often reduces the mobility of the shoulder and causes pain that becomes chronic. A poorly stretched shoulder leads to lymphoedema, which leads to depression and reduced ability to work. Effective analgesia is a prerequisite for physiotherapy, which is an important part of preventing the late effects of surgery. Fatigue, which is still a major medical problem that later very often affects daily life and work capacity, is a major problem facing almost every patient during and after cytostatic treatment. Systemic cancer treatment can also lead to impaired cognitive function, ovarian failure with or without menopausal symptoms, sexual problems, decreased libido, weight gain, neuropathy (after taxane treatment), alopecia, osteoporosis or osteopenia, heart failure (after anthracycline treatment), many other problems, and secondary malignancies.

COBISS.SI-ID: 3407227Based on the concept of comprehensive rehabilitation, the Multidisciplinary Group for Integrated Rehabilitation of the National Cancer Management Program has prepared the “Recommendations for the comprehensive rehabilitation of breast cancer patients” for 14 different sets of consequences of oncological treatment. It was prepared on the basis of foreign guidelines and domestic expertise. Following the review of the foreign guidelines, the multidisciplinary working group also prepared two triage questionnaires that, with questions in 14 different sets of questions, which help the patients to self-assessment of their problems.
