Abstract
In 2004 Slovenian renal registry database was founded, which included extensive demographical and clinical data from all slovenian patients treated by renal replacement therapy (dialysis and transplantation). Data from year 2004 were joined to European EDTA/ERA registry. Special focus in registry database will be dedicated to vascular access in hemodialysis patients. Hemodialysis patients represent the majority of patients treated with renal replacement therapy in Slovenia. Vascular access database is already included in the present Slovenian renal registry. The aim is to extend it with more data. Slovenia has excellent results in the field of vascular access for hemodialysis comparing to other countries, however, our goal is further improvement. All patients with suboptimal vascular access in Slovenia will be identified, possible causes analyzed and strategy for improvement implemented. Annual Renal registry report will evaluate the results of this strategy. Along with the epidemiological study of vascular access in Slovenia, a series of clinical studies on vascular access in high risk group (children, elderly and patients with failing kidney graft) will be performed. 1st clinical study. Objective: to assess forearm and arm arteries and veins in children and adolescents with end-stage renal disease by ultrasonography. Working hypothesis: Preoperative mapping of arm and forearm vessels in children and adolescents shows that majority of them have adequate forearm vessels for arteriovenous fistula construction. With high quality mapping successful AV fistula construction and use is achieved in > 90% of patients. Methods: prospective clinical study with use of ultrasonography/Doppler. 2nd clinical study. Objective: to assess forearm and arm arteries and veins in elderly patients with end-stage renal disease by ultrasonography. Working hypothesis: Preoperative ultrasonography mapping of arm and forearm vessels in the elderly shows that majority of elderly patients have adequate forearm or arm vessels for arteriovenous fistula construction. The main problem are calcinations in artery wall. Methods: clinical study with use of ultrasonography/doppler. 3rd clinical study. Objective: to assess forearm and arm arteries and veins in patients with failing kidney graft. Working hypothesis: Preoperative ultrasonography mapping of arm and forearm vessels in patients with failing renal graft shows extensive calcifications in forearm and arm arteries. Patients receiving steroid therapy have additional risk for primary arteriovenous fistula failure. Methods: prospective clinical study with use of ultrasonography/Doppler.
Significance for science
Vascular access is of utmost importance for performing hemodialysis (and other procedures requiring extracorpopreal circulation). Serie of our studies is focused on the especially demanding patients subpopulation concerning vascular access: children, elderly and patients dialyzed for 30 years and more, patients after kidney transplantation). We believe that results of our studies will contribute to the knowledge of the field. In addition, ,they will contribute to further affirmation of the slovenian model for vascular access for hemodialysis for the most demanding patients.
Significance for the country
Slovenia is welknown internationally for the high quality of vascular access for hemodialysis, provided by interventional nephrologists, which is the practice not common in the world. With intensive research on vascular access, which is complementary to the intensive clinical work, we develop this area of sicence and clinical practice and become the referral center for other ocher countries, who send us nephrologist and the most demanding patients.
